SPECIALIZING IN NEURODEVELOPMENTAL ASSESSMENTS AND DIAGNOSTIC CLARIFICATION

We offer the following assessments for teens and adults age 13 and older. (Please note autism evaluations are only offered for individuals who are verbally fluent):

  • Neurodevelopmental (ADHD, Learning Disability & Autism) 
  • Cognitive (Intellectual disability & Giftedness)
  • Psychodiagnostic (Mood, Anxiety, Trauma, OCD, psychosis, personality dysfunction)
  • Diagnostic clarification (identify/disentangle multiple or confusing diagnostic presentations)
  • Comprehensive (Full assessment of multiple conditions to identify any present diagnoses)

GETTING STARTED

If you are seeking an assessment or aren’t sure but would like to discuss it, the first step is to complete a request for services referral form. You do not need a referral from your therapist or doctor’s office, you may self-refer.  

To get started, fill out and submit your information via our secure referral link below. You may contact our office to have this emailed to you as well. If you are a provider interested in referring current or future patients, you may copy this link and re-use it an unlimited number of times. the more thorough you are in completing the form, the more helpful we can be.  We will follow up no later than one week after we receive your form, though we will try to respond the next day if possible.

THE PROCESS

The following is a step by step from start to finish of the assessment process. This includes information about how to make your assessment as efficient as possible to minimize cost.

FIRST

PORTAL SET UP & PAPERWORK

Once we receive your intake packet (ID, Insurance card, Billing info form, ROI from your PCP) we will create a patient portal for you where we will share consent documents, questionnaires about your mood and functioning and other information we need for your assessment along with instructions and a check list. Once you complete this step  you will be ready for scheduling

SECOND

REVIEW AND SCHEDULING

With the paperwork complete we will schedule a time to review everything with you on the phone, schedule an intake appointment as well as a testing and assessment appointment, and answer any questions you might still have. Your intake appointment is a 90-minute telehealth appointment (sure, you may come in person if you prefer!). The testing and assessment appointment varies from three to five hours and is in person. 

Testing appointment: Many diagnostic evaluations require at least one in person testing appointment at our outpatient clinic downtown. Measures are in place to adhere to the CDC recommendations for social distancing to mitigate risk associated with contracting COVID-19. These include but are not limited to wearing a mask, maintaining adequate distance, sanitation procedures, and air filtration.

You may be asked not to take certain medications and abstain from the use of stimulants such as caffeine or nicotine prior to testing on the day of your appointment. If you use cannabis, you will be asked to abstain for one week prior to testing if possible. In some (but not all) cases, testing will need to be cancelled and rescheduled if you forget. 

Other remote testing may be completed with video proctoring. This means you will complete the testing from home and the provider will join via telehealth to administer the testing remotely. This will be discussed with you and scheduled during your intake appointment. It may also be scheduled following your testing appointment based on other information. The provider will discuss all decisions and suggestions for testing records or other relevant concerns throughout the assessment process. We strive to be as transparent as possible.

THIRD

INTAKE

After your intake appointment, the examiner will talk about the kind of testing you will complete at your next visit and tell you a little bit about what to expect. You may (or may not) be given questionnaires or other objective measures to complete in the meantime. 

It is important to have information from a variety of different sources to complete an assessment. A lack of information may limit the provider’s ability to answer the assessment question and provide evidence-based recommendations for the most appropriate treatment or services. It may also result in increasing the cost of the assessment in term of both time and money because we have to spend more time trying to get the information we need. 

FOURTH

TESTING & ASSESSMENT

You will attend your testing and assessment appointment. Here you will complete any testing and additional interview questions as needed. More complex assessments sometimes require two testing and assessment appointments (for example, multiple diagnoses or assessment for both ADHD and autism spectrum differences); however, many complete the process with one in person visit and one telehealth intake. 

LAST

FEEDBACK

Once everything we have everything, we need including records and information from collaterals, and your report is complete, we will reach out to schedule a 45-60 minute feedback session that takes place via telehealth. Here you will receive information about the conclusions of the report as well as education and recommendations for treatment and other helpful supports. You will be given the opportunity to ask questions and can attend alone or with a loved one. You will be provided with an assessment report and this can be forwarded to any one else you wish with a signed release of information.
